What should you bring to your appointment at Interventional Spine and Pain, P.A.

  • Patient registration forms (you may download from our site, or we will fax you)
  • All previous and relevant imaging studies with imaging report (X-ray, MRI, CT-scan, Ultrasounds)
  • Previous medical records
  • A list of questions you may want answered about your condition and/or treatment
  • A list of all current medications
  • Insurance Card
  • Drivers license
  • Proper Co-pay amount (this will be a specialist visit, so please consult your insurance card for the correct specialist co-pay)

Insurance Information

We accept all Major private insurances, with the exception to Medicare Advantage plans/Replacement plans or Medicaid.

Medicare recipients must have a supplemental as a secondary insurance in order to become an established patient, and must also have a referral from a referring provider.
